Attention parents and athletes: It is that time of year when many of you start thinking about summer camps and next year's sport seasons. It also means it is time to get an updated athletic physical. As a matter of policy, we would like to remind parents/guardians and athletes of the following:
• The Dodge City High School activities department requires athletes to complete a new physical each academic year.
• New physicals must be complete after May 1 for the upcoming academic year.
• Physicals may only be signed by a registered medical doctor, doctor of osteopath or a physician's assistant.
• Those physicals completed by an advanced registered nurse practitioner must be co-signed by a physician.
• The activities department requires a current physical (completed/dated after May 1) be on file to participate in summer weights and summer athletic camps.
This year, the activities department, in conjunction with some of our local physicians and medical providers, will host the third annual "Physical Night" on Monday. “PPEPP-UP '08” (Pre-participation examination physicals program uniting providers) physicals will be done from 6-9 p.m. at the DCHS Fieldhouse. The cost for the athletic physical will be $10. A first-come, first-serve format will be used. Copies of completed physicals will be available if requested.
Those athletes not in the door and registered by 9 p.m. will need to schedule a physical with their own medical provider. This is a once-a-year opportunity for all of our Dodge City athletes entering grades 7-12.
Any Dodge City athlete entering grades 7-12 planning to participate in a sport during the 2008-09 school year should take advantage of this unique opportunity. In order to participate in “PPEPP-UP '08”, athletes must do the following:
• Pick up a physical form in the DCHS or Dodge City Middle School activities office before Monday. (Physical forms will be available at the registration table; however, forms require a parent signature and insurance information, so please be prepared.)
• Have the front back pages completely filled out (including the required parent signature, insurance information and medical history section. Failure to complete the required sections will result in NOT being able to complete the physical.)
• Have the $10 registration fee. (Make check payable to DCHS.)
• Arrive at the DCHS Fieldhouse activity entrance at the times listed below on Monday in order to speed the process.
